- Animal Pet Vaccination Records

- Make sure that the front page of the record has the name of pet, breed, species, age/birthdate, sex, marking, name of owner.
- The vaccination records should also show the pet’s name, date, weight, vaccine sticker or rabies vaccine used, manufacturer, lot and serial no, and the signature of veterinarian with their license no., date of expiry, TIN no., and PTR.
- Minimum age of 3 ½ months old for all dogs and cats
- Your pet is vaccinated against Rabies not less than 14 days or more than 1 year prior to the date of travel.
- 1 VHC, 1 Destination, 1 Shipping permit policy

- When transporting more than 1 pet (dog/cat) in one application with the same destination, the entry details and description of each pet must be provided
- Personal details of pet/s (species, breed, sex, color, age, name of owner/exporter from country of origin and their address)
- Proof of Microchip number (must embedded in the skin of the dog/cat)
- Vaccination and deworming records of pets, dog/cat (must be original or certified true copy)
- Photograph/s of pet, dog/cat
- PET Passport of pet dog/cat, if applicable
- Minimum age of pets, dog/s and cat/s must be at least 4 months;
- Dog/s and/or cat/s to imported must have updated Rabies vaccinations, not <14days or more than 1 year prior to SPS Applications;
- Dog/s must have also have DHLPPi vaccination while cats should have vaccinations against feline panleukopenia;
- Dog/s and/or cat/s to imported must all have been dewormed for internal and external parasites;
- Import maximum of 3 animals once a year, or import maximum of 3 animals, more than once a year provided same animals identified through its microchip numbers, are imported.
- Copies of approved SPSIC
- Printed copy of International Veterinary Certificate and Veterinary Health Certificate
- Airway Bill
